PelviQuest for Clinicians: Structured Endometriosis Intake, Examination and Decision Support

PelviQuest gives gynecologists, urogynecologists and pelvic pain specialists a standardized way to capture what the diagnostic delay in endometriosis usually loses: a complete, consistent symptom history across organ systems and the cycle, a structured examination, and a longitudinal record between visits. The platform scores the combined picture with a model trained on surgically confirmed cases and produces a report both patient and clinician can act on. It is decision support, built by endometriosis and pelvic nerve surgeons, for the first visit through to surgery and follow-up.

The clinical problem

Guidelines from NICE and ESHRE now endorse clinical diagnosis of endometriosis and empirical treatment without laparoscopy. In practice, the limiting factor is the quality and consistency of the history and examination on which that diagnosis rests. A fifteen-minute visit, an unstructured narrative and a patient who has learned to under-report pain produce a record that neither supports a confident diagnosis nor allows comparison over time. PelviQuest standardizes the inputs.

Patient-side: the standardized intake

  • A standardized questionnaire covering dysmenorrhea, non-cyclical pelvic pain, deep dyspareunia, dyschezia and bowel pattern, urinary symptoms, fatigue, back and leg pain, menstrual and obstetric history, fertility and family history, completed before the visit.
  • Daily tracking of pain, bleeding, bowel, bladder, mood and medication, so the cyclical pattern is in the chart rather than reconstructed from memory.
  • Medication list with each dose marked taken or skipped, an effectiveness rating and side-effect notes entered by the patient.
  • A patient-facing PDF report summarizing the record, which patients can bring to any clinician.

Clinician-side: structured examination and visit documentation

  • An examination wizard that records pelvic findings as discrete fields: tenderness by site, nodularity, uterine position and mobility, adnexal findings, pelvic floor tone and nerve tenderness, with conditional sub-questions driven by positive findings.
  • Per-visit care cycles so a pre-operative evaluation, a surgical episode and post-operative follow-up are documented as distinct, comparable events.
  • Immutable visit notes with on-demand PDF export for the chart or for referral.
  • A 0 to 100 screening score for endometriosis derived from the questionnaire and examination, shown alongside its inputs, never in isolation. It has not been validated as a diagnostic test.
  • Clinic-wide patient visibility with assigned-provider pointers, so a multi-provider practice sees one record per patient.

Practice operations

  • Front-desk scheduling with provider and room-based calendars, booking rules, and a review-and-confirm queue for patient-initiated requests.
  • Read-only synchronization of appointments from DrChrono, so the PelviQuest calendar reflects the practice schedule without duplicate entry.
  • Role-based access for owners, providers and front-desk staff, with invitation-based onboarding and the ability to revoke access.
  • Patient enrollment by a clinic-specific code, so patients who find the app independently are connected to the right practice.

Security and compliance

  • Encryption in transit and at rest, including encrypted identifiers for patient records.
  • Role-based access control and audit logging of access to protected health information.
  • Short-lived clinician sessions with multi-factor authentication.
  • Designed to meet HIPAA requirements; business associate agreements available for clinical partners.
  • Research use restricted to de-identified or aggregated data.

The research partnership

The model behind the screening score was developed from a single surgical practice in which standardized pre-operative data were matched to intraoperative findings. Its generalizability depends on data from other practices and populations. PelviQuest is seeking endometriosis surgeons and pelvic pain clinics to participate in external validation: contributing standardized intake and examination data matched to surgical outcomes, under appropriate ethics approval and data agreements, with the goal of a tool that performs reliably wherever it is used.

What PelviQuest is not

It is not an electronic health record and does not replace one. It is not a diagnostic device and does not make or recommend a diagnosis or treatment. The score is one input to a clinician’s judgment, intended to raise the floor on how early and how consistently endometriosis is considered, not to set a ceiling on evaluation.

Getting started

Physicians can register through the physician registration page. Practices interested in the clinic platform or in the research partnership can leave their details in the form below, and we will arrange a walkthrough. Onboarding includes configuration of providers, rooms and booking rules, and training for front-desk staff.

Research partnership

The clinic platform, free for research partners

We provide the PelviQuest clinician platform to clinics at no cost, with no licence or subscription fee, in exchange for collaborating with us on research.

The collaboration is a joint study, not a data feed. We agree on a research question together, your practice keeps using the standardized intake and examination your clinicians already complete in the platform, and we analyze the de-identified results together, with shared authorship on anything that is published.

Our data commitments

  • We do not ask for, and do not want, identifiable or sensitive patient information. Research uses de-identified data only.
  • Data are used solely for the study we agree on together. They are never sold, shared with, or licensed to third parties.
  • Participation runs under ethics (IRB) approval and a written data agreement, and either party can end the collaboration at any time.
  • Your practice remains the custodian of its own patient records.

Interested? Leave your details

We will get back to you to talk through the platform and the study.

Frequently asked questions

Does PelviQuest replace my EHR?
No. PelviQuest is a specialized intake, examination and decision-support layer for pelvic pain and endometriosis. It currently reads appointments from DrChrono and exports visit notes and reports as PDFs for your chart.
How is the endometriosis screening score generated?
From the standardized patient questionnaire and the structured examination findings, scored by a model trained on patients whose endometriosis status was confirmed at surgery. The score is displayed with its inputs and is decision support, not a diagnosis.
How can my practice participate in validation?
Leave your name, email and phone in the research partnership form on this page. Participating practices receive the clinic platform at no cost. Participation involves collecting the standardized intake and examination data and matching them to surgical findings under ethics approval and a data agreement, using de-identified data only.
